HOW TO SUBMIT A REQUEST:
1. All applications must be mailed to the BOM Counseling Officer. Do not send requests to the Conference Center, the BOM President, District Superintendents, or any other officer. The current Counseling Fund Officer is:

2. You do not need to express the reason for your desire for Counseling Fund reimbursement. Please give the following information.
a. Your name, SS#, and Conference standing
b. Your request for reimbursement, indicating your health insurance and denial of coverage. The Counseling Officer reserves the right to request written confirmation from the health care provider.
c. Indicate if it is for you or a family member (no names).
d. Provide the name, address, and telephone number of your counselor. If possible, confirm that they are certified.
e. Outline any outstanding circumstances that may provide greater necessity for funds. (e.g. frequency of sessions, family financial circumstances, etc.) Please share only the basic information. If more detail is needed, you will be contacted by the counseling fund officer.
